Health Records Fixed the Plumbing but Kept the Walls Up

Technical standards for clinical data exchange work reliably now. The problem is that healthcare providers still profit from keeping patients inside their own walls.

Standardised interfaces are no longer the bottleneck in digital health. Modern application programming interfaces and shared clinical vocabularies make medical records transportable across systems. The protocols perform as expected when two clinics decide to connect.

Yet routine transfers still fail or require manual intervention. The failure is rarely technical. It occurs because the institutions holding the records have little financial reason to give them away.

The triumph of modern protocols

For two decades, health software vendors blamed incompatible databases for fragmented care. That argument lost its technical grounding with the widespread adoption of modern data exchange frameworks. Structured clinical summaries now translate cleanly between different software suites.

Cloud infrastructure makes the transmission instantaneous and relatively cheap. Validation tools catch syntax errors before records cross network boundaries. On an architectural diagram, the problem of clinical interoperability looks entirely solved.

Friction as a retention strategy

Hospital systems run on volume and referral capture. When a patient moves to a competitor, revenue leaves with them. Frictionless data movement makes leaving easier, which conflicts directly with institutional business goals.

Data liquidity threatens patient retention in competitive healthcare markets. Consequently, organisations comply with open standards only to the minimum required degree. They permit inbound data while quietly burying outbound export functions behind legal reviews or technical fees.

Even software suppliers protect their market share by charging excessive configuration costs for standard connections. They deliver open interfaces on paper, but turn implementation into an expensive bespoke project. The code conforms to law, but the price tag deters integration.

The limits of technical mandates

Regulators assumed that setting common data formats would force open the market. They treated an economic stand-off as an engineering glitch. Mandating standard protocols cannot produce cooperation when market rewards favour isolation.

True coordination only appears in single-payer environments or integrated care networks where one entity covers both treatment and long-term costs. Outside those models, every transfer remains a concession to a rival. The pipes are ready, but the business case for keeping them dry remains intact.
